Role of Immediate Loading Continues to Evolve to New Applications

The concept of immediate loading has been within the domain of implant dentistry for about the past 20 years. As this approach moved from full-arch to more single-tooth applications, the scope of expanded services was made available to more providers, and the clinical approach has provided enhanced benefits to patients. As the therapeutic approach has evolved, there is more interest in reducing the complexity of care. One approach has been to use fewer implants. Mezzari et al (2019) presented a case series over 5 years using three implants for the edentulous mandible and demonstrated satisfactory implant outcomes, although a high level of prosthetic complication was reported. Biologically, the response to immediate loading was evaluated by Denardi et al (2019). Using a systematic review approach, the authors observed that ongoing bone remodeling occurred around oral implants in a manner similar to the healing models anticipated around socket healing. Meijer et al (2019) also evaluated the impact of the residual wall thickness. The study emphasized the key role in site evaluation and residual bone thickness for predictable healing. The long-term outcomes are also important, and Nicolau et al (2019) presented 10-year data on one implant system that was immediately loaded and showed very predictable long-term outcomes. For early and immediate loading in the posterior arches, Rossi et al (2018) showed strong outcomes for short 6-mm implants at 10 years for posterior tooth replacement. Hattingh et al (2018) followed with a clinical evaluation for the “ultra-wide” implants that have come on the market for molar replacement therapy. Within the confines of this particular study, the authors indicated good success. Implant therapy continues to evolve in implant design, therapeutic approaches, and application to a wider array of clinical applications.

Augmented Reality in Microsurgical Endodontics: The Future of Endodontic Treatment

Endodontic specialists Bertrand Khayat, DDS, MSD, and Guillaume Jouanny, DDS, first met in the classroom while Dr Jouanny was an undergraduate student in Dr Khayat’s class. What began as a mentorship forged over deep commitment to the profession soon blossomed into a decade-long collaboration between the two clinicians, surgeons, and lecturers. Recently, they have combined their knowledge into the textbook Microsurgical Endodontics.

“This book can be used as a reference for simple as well as for more complex indications.”

“This book is designed to help every practitioner, general dentist, and endodontist to perform endodontic microsurgery,” they say. “Every step is described in detail from the anesthesia to the sutures, and special emphasis has been placed on the clinical aspect of the procedure with plentiful illustrations and clinical examples. This book can be used as a reference for simple as well as for more complex indications.”

Microsurgical Endodontics brings to light the use of endodontic surgery as a reliable, high-success treatment solution that, until now, has too often been overlooked. Given the breakthroughs in recent years, endodontic surgery has advanced to the point where it rivals conventional practices. In this book, endodontic surgery is finally being appreciated for its effectiveness as a treatment option, especially when conventional retreatment is not ideal.

“It is now possible to perform endodontic surgeries with a success rate of more than 90%.”

“Microsurgical endodontics is not an apical resection anymore,” the authors explain. “The use of the operating microscope, the evolution of the ultrasonic tip, and new materials allow for a better prognosis. It is now possible to perform endodontic surgeries with a success rate of more than 90%. In the peri-implantitis era, it is obvious that mastering endodontic surgery will be a great asset to save more teeth.”

Helping clinicians excel in their daily practices is a major priority for Drs Khayat and Jouanny. With the advent of recent advancements in technique, the authors recognized a need to fill.

“We read all the literature that was already published on this particular topic, and our conclusion was that the clinical side was very often left out,” they say. “We decided to write a book that would summarize the updated scientific evidence available and emphasize the clinical part of the procedure. We presented more than 100 cases with follow-ups as far as 12 years. Every step is precisely described, so clinicians can use the latest techniques to the benefit of their patients. We decided to show not only the classical before-and-after radiographs, but also the tips and tricks that are not usually shared in the literature.”

The authors cite chapter 7 as a perfect example of their manifested efforts to make critical steps easy to follow, especially the “crossed incision technique.”

“The first part of the chapter is devoted to the anatomy,” they explain. “Soft tissues are clearly described with precise 3D rendering drawings and clinical pictures. The second part of the chapter addresses the technique of incision. Every step is broken down and conceptualized so readers can reproduce the gestures easily in their offices.”

To reinforce the material presented, Microsurgical Endodontics takes QR codes to the next level by incorporating the use of augmented reality, thereby providing readers with an enhanced visual experience. Readers can download an app for iOS or Android that allows them to access videos handpicked by the authors. These supplementary videos bring select images in the book to life when you simply hover a smart device over each image, and you don’t even need an Internet connection.

“The augmented reality feature is a new step in education,” the authors say. “It is now possible to precisely visualize the gestures explained in writing. Different key movements with a blade, an elevator, or an ultrasonic tip are shown in short videos so that the reader better understands how to perform the procedure.”

Microsurgical Endodontics incorporates the use of augmented reality through an app, so readers can access supplementary videos offline that bring select images in the book to life.

The authors felt compelled to incorporate augmented reality into their book when they realized something was missing from its detailed diagrams. They call this missing element the “live demonstration effect.”

“The augmented reality feature is a new step in education.”

“The idea behind the augmented reality is to give the feeling of a live demonstration directly through your smartphone or tablet,” they explain. “Augmented reality is even better than a live demonstration because it is possible to visualize one particular clinical gesture, to focus on small details, and to repeat it indefinitely wherever you are, as long as you have your book with you.”

Even if you left your book at the office, if you have an Internet connection, all the videos are accessible through a website link within the app for a traditional video-viewing experience in a pinch.

“We believe that augmented reality is definitely the future of education,” the authors predict. “It is already possible to virtually navigate into dental canals, to use augmented reality models for training, and to even perform surgeries in augmented reality.”

This enriched reading experience is just one part of the authors’ vision for the book and the successes in endodontic treatment they anticipate resulting from its use.

“Our goal is that every dentist understands the wide range of indication of endodontic surgery,” they emphasize. “We hope that students, general dentists, and experienced specialists will find relevant information and that this book will help our colleagues take a step further in endodontics and start to include endodontic microsurgery in their daily practices.”


Bertrand Khayat, DDS, MSD, received his DDS at the University of Paris. He obtained his Certificate in Endodontics and his MS at the University of Washington in 1988. A pioneer in the use of the microscope in endodontics, he also designed several surgical instruments including ultrasonic tips for surgical retropreparation. In addition to his extensive experience and expertise in endodontic surgery, he lectures internationally while maintaining his private practice in conventional and surgical endodontics in Paris.

 

Guillaume Jouanny, DDS, received his DDS at the University of Paris. He earned his Certificate in Endodontics from the University of Pennsylvania in 2015, where he received comprehensive training in microsurgical endodontics. He returned to France where he teaches at the university level. In addition to his private practice in Paris, he lectures on surgical endodontics, among other endodontic topics, worldwide.

 

 

The authors have collaborated for the last decade on multiple publications and a textbook dedicated to endodontic microsurgery in French. Together, they founded a clinical training program that welcomes international dentists seeking mastery in surgical endodontics.

Bringing Q&A from Classroom to Bookshelf: A Guide to Soft Tissues and Pink Esthetics in Implant Therapy

Today, a website’s FAQ page is often the first place we consult for answers. Prolific lecturers and clinicians Daniele Cardaropoli, DDS, and Paolo Casentini, DDS, capitalized on this staple of the Internet in their new book, Soft Tissues and Pink Esthetics in Implant Therapy.

Each chapter is devoted to a specific topic and opens with a series of concise, punchy answers to readers’ top questions.

“After our 20 years of experience in postgraduate dental education dealing with colleagues from various countries,” says Dr Cardaropoli, “we found that the needs are always the same: to receive clear answers to specific clinical questions.”

The book’s unique structure is made up of three simple parts: questions, answers, and clinical case studies. Each chapter is devoted to a specific topic and opens with a series of concise, punchy answers to readers’ top questions. The second part of each chapter includes detailed case studies that illustrate a variety of clinical situations familiar to clinicians who deal with similar cases in their daily practices. This innovative structure provides for a clear, swift reading experience.

“We selected a number of questions regarding the topics covered in each individual chapter, following the structure of the FAQ,” Dr Cardaropoli explains. “In this way, we avoid the usual ‘boring’ introduction and instead push readers into the clinical practice from the beginning. For this reason, we think the use of the textbook will be more fluent for our colleagues. They will find several options of treatment for each clinical scenario, with a detailed description of the treatment plan and the surgical approach, as well as the prosthetic rehabilitation in implant dentistry. The textbook, with its vast number of images (more than 3,500 distributed across 11 chapters) will be a tool to guide the implant treatment plan—a reference to consult before surgery.”

Drs Cardaropoli and Casentini can be found lecturing 42 weeks each year, all around the world. Addressing colleagues’ needs through question-and-answer sessions at these lectures is their main focus. They drew from these experiences to strategically select the top questions for inclusion in Soft Tissues and Pink Esthetics in Implant Therapy.

“We have witnessed our colleagues become increasingly interested in obtaining information to solve their surgical needs and improve their treatment plans,” says Dr Cardaropoli. “Obviously, all the information that is provided in the text is based on what we call scientific evidence, but at the same time we tried to select only the data that can really impact the clinical practice. Generally, for each topic, the questions that are asked during the conferences and courses are very similar to each other, regardless of the latitude, and we have thus had the opportunity to select the most frequent—the most significant. So, we have tried to rationalize this approach within the various chapters, with first an introductory part consisting of the questions with their answers, followed by the selection of numerous clinical cases (more than 90 in total) completely described both in the surgical and prosthetic phases.”

Dr Cardaropoli is continuing his lecture series with a stop at the Dr H.W. Haase CE Center in Batavia, IL, this October. His 2-day hands-on workshop, eponymous with his new book, will cover different surgical and prosthetic approaches for single- and multiple-tooth rehabilitations, from immediate implant placement to ridge preservation and horizontal and vertical bone regeneration to soft tissue augmentation.

Example of connective tissue graft positioning technique performed at the same time as immediate implant placement. The graft is positioned between the connective tissue and buccal cortical bone, if possible in a supraperiosteal position, using a flapless approach or by raising a flap. This graft does not obviate the need to pack the gap between bone and implant with a biomaterial.

“During the workshop, I hope to transmit the knowledge and experience I have accumulated over many years,” Dr Cardaropoli anticipates. “Starting from the very next day, participants will be able to increase the number of patients positively treated, widening their therapeutic options. Specifically, together with the description of the techniques and biomaterials used for bone augmentation, an up-to-date evaluation of the role played by the soft tissue (both from the functional and esthetic point of view) will be assessed. A decision tree will be also presented to guide the clinical selection between ridge preservation, ridge augmentation, and GBR following tooth loss. Finally, contemporary techniques for soft tissue reconstruction will be described.”

Smooth crossover of the Q&A structure between textbook and workshop is a feature of Dr Cardaropoli’s teaching style.

Daniele Cardaropoli, DDS, graduated with honors with a degree in dentistry and dental prosthetics and subsequently pursued postgraduate studies in periodontology at the University of Turin. He obtained postgraduate diplomas in periodontology and implantology from the Harvard School of Dental Medicine and in biomechanics from the University of Siena in Italy. In addition to his private practice in Turin, Dr Cardaropoli is the scientific director of the Institute for Professional Education in Dentistry (ProEd) and head of oral implantology at the Sedes Sapientiae Clinic. He is a member of the editorial board of The International Journal of Periodontics & Restorative Dentistry and a reviewer for several journals. Dr Cardaropoli lectures widely and is also the author of numerous publications. He is the recipient of both the Henry M. Goldman Prize for clinical research from the Italian Society of Periodontology and Implantology (SIdP) as well as the Italian Society of Orthodontics (SIDO) National Award in Clinical Orthodontics. Dr Cardaropoli is an active member of the SIdP, the European Federation of Periodontology (EFP), the Italian Academy of Osseointegration (IAO), and the Academy of Osseointegration (AO) and an international member of the American Academy of Periodontology (AAP).

Paolo Casentini, DDS, earned his degree in dentistry from the University of Milan and subsequently attended the oral surgery and implantology program at San Paolo University Centre in Milan. He currently maintains a private practice in Milan, with a particular interest in regenerative surgery and esthetic treatment. Dr Casentini is the author of numerous journal articles in the fields of implantology and regenerative surgery as well as the online education channel. He is also the co-author of nine books on oral surgery, implant surgery and prosthetics, and advanced implantology. He has lectured in more than 40 countries and is a fellow and president of the Italian Section of the International Team for Implantology (ITI), director of the ITI Milan 1 Study Group, and an active member of the IAO and the SIdP.
